Registered & Licensed Practical Nurses
All items below must be current and on file prior to placement
RN or LPN license (active)
Level 2 Background Screening on file with AHCA
Valid CPR card AHA or American Red Cross
Communicable disease clearance (within 6 months)
1-Hour HIV/AIDS Certificate
30-Hour Infusion Therapy Certificate (LPN only)
Professional Liability Insurance (recommended)
Current Florida Driver's License
Current Automobile Insurance & Vehicle Registration
Home Health Aides & Certified Nursing Assistants
All items below must be current and on file prior to placement
CNA License in Florida (or HHA certificate 40hr min)
Level II Background Screening on file with AHCA
Valid CPR Card AHA or American Red Cross
Communicable disease clearance (within 6 months)
1-Hour HIV/AIDS Certificate
1-Hour Alzheimer's Training Certificate
2-Hour Self-Administration of Medication Certificate
Professional Liability Insurance (recommended)
